Anatomy of the Eye

Your eyes are a crucial feature in your anatomy because it enables the sense of sight. Sight enables us to absorb much more stimuli that any of our other four senses and that is why it is utilized in almost every activity that we perform with the exception of sleeping. The process in which the eye functions is a truly beautiful aspect of nature in its full form of technological brilliance. For example imagine that there is an inanimate object in front of you. In order for you to see this it is important that a certain amount of light enters your eye through the cornea. The cornea is the clear round dome at the front of the eye. The light then flows through the cornea and in to the pupil which is the circular portion at the center of the colored iris which is the colored part of the eyeball. Changes in the amount of light correlates with the size of the pupil. The darker it is the larger the pupil gets in order to retain more information and the lighter it is the more constricted the pupil becomes because it is unnecessary for the pupil to take in an excessive amount of light as it is already present in the surroundings. This process is referred to as the pupillary light response.

When light enters the cornea in the initial stages, they are converged and then bent further by the crystalline lens which is located immediately behind the pupil to a nodal point that is located behind the inverted surface of the lens. Here the image focuses in a reversed manner and our brains turn the image the ‘right side up’ for us. It is therefore safe to say that we don’t see with our eyes but with our brains though our eyes are the instrument that enables the visual process to take place. Cataract Treatment

A cataract is generally a cloudiness that affects the usually transparent crystalline lens of the eye. The lens of the eye is what enables us to focus on objects effectively and a cataract hinders this process. Even though surgery for cataract can be delayed for a long period of time and sufferers are able to see with the help of visual aids, surgery is still the only known cure for this visual ailment up to date when the disease is in its latter stages. There are three basis and renowned surgery for cataract available in the market today. They are; Phacoemulsification, Extracapsular cataract surgery and Intracapsular cataract surgery.

Glaucoma

Glaucoma is a cluster of eye diseases which in most cases produce increased pressure within the eye which in turn leads to an increase of fluid levels in the eye. This over time causes damage to the optic nerve and results in the blurring or loss of vision over a period of time. Damage to the optic nerve makes the retinal ganglion cells in your eye undergo a slow process of deterioration called apoptosis. This is what leads to permanent vision loss in certain glaucoma cases.

Macular Conditions

Macular degeneration also referred to as ARMD is considered to be one of the most prevalent vision ailments in individuals over the age of fifty in the United States. This being said it is important to note that we require our macula to see detail and the lack of this result in not complete vision loss but distorted middle vision instead. The macula is a minute section of the retina that has the highest resolution; a feature that enables us to see great detail. Macula is a Latin word that means ‘spot’ and this is the ‘spot’ that we use to do most of our day to day activities that require precision, such as; reading, driving and even recognizing the faces of your loved ones. In order for you to imagine what your vision would be like without a properly functioning macula, visualize having a blank right in the middle of your eye sight but being able to see around it? Keratoconus Treatment

Retinal Conditions

Some retinal eye disorders are;

Latest Technology for Lasik’s Patients Schwind Amaris 750 SThe 750 Hertz speed of the Amaris Laser has many advantages over all the other laser generations and the other competitors in the market.
In this report, we will present you the main features and clinical based advantages of your laser machine.
First we need to recognize that the human eye is a very delicate organ, and very sensitive to energy or heat.
So when you expose the eye to heat for a short time, you will have much better clinical outcome, while exposing the eye to energy for longer time is more damaging.
And the less heat (energy level) you put on it, the better is the clinical outcome as well.
So the energy exposure time and the energy level are VERY ESSENTIALS for your patients long term wellbeing.
1- Time of Treatment:
The Amaris 750 needs 1.5 seconds to correct 1 Diopter of myopia, while the 500 Hertz model needs 2.5 seconds. That means the new Amaris takes 66% less time to treat 1 Diopter.
To correct -5 Myopia for example with the 500 Hertz machines, it will take around 12.5 seconds of laser energy hitting the cornea of the eye, while the Amaris 750 will take 7.5 seconds. So the time is hugely decreased, and this is the first criteria of better clinical outcome.
If you take the average treatment time of the other machines in the market, you will find the the Amaris 750S has 3 times faster treatment time. (the average of the industry is 4.6 seconds)
In Hyperopia, the Amaris 750S takes 2.7 seconds to correct 1 Diopter while the average time of all other machines in the market is 8.6 seconds. And that is a huge radical change in technology.

The Amaris 750 has a new technology not present in any of the competition and it’s called, Automatic Fluence Level Adjustment. The Energy begins at 450 mj/cm2 and it ends at 160 mj/cm2 (mj = millijoule) while with other machines, during all the treatment the energy level is constant around 400 mj/cm2. And this will create as well smoother cornea (smoother cornea means better quality of vision).
So both Point 1 and Point 2 lead to much better clinical outcome and to much faster recovery time and minimize enormously the risk of dry eyes after the LASIK treatment.
3- The Hot Points or Distribution of Energy on the Cornea Surface:
Schwind invented the “Intelligent Thermal Effect Control” feature and it ensures that the laser pulses are distributed in a thermally optimized, dynamically adapted way, which gives the individual position on the cornea sufficient time to cool off. ITEC is clearly superior to the otherwise customary static processes: Studies using an infrared thermographic camera have proven that the corneal tissue only heats up by a maximum of 4° Celsius, even at high ablation speeds.
So in the language of the patient, the “Intelligent Thermal Effect Control” will keep the eye cold enough and that will also help in minimizing the risk of dry eye syndrome after Lasik treatment.
Clinical studies with the Amaris 750S showed no trace at all for dry eyes after treatment.
The most common complaint from patients due to refractive surgery is the incidence of “dry eyes”. According to an American Journal of Ophthalmology study of March 2006, the incidence rate of dry eyes from LASIK after the six month post operative healing period was 36%.The FDA (Food and Drugs Administration) website states that "dry eyes" may be permanent.
That is why the short time of Amaris 750S treatment, and the “Intelligent Thermal Effect Control” preserves the corneal nerves and minimize their damage.
4- The wide optical zone of Treatment:The range of the Optical Zone with the Amaris is from 4 to 10 mm. And that helps to minimize the glare and hallow that can be detected in the vision of your patients at night with the regular Lasik.
Starbursting refers to rays or fine filaments coming off lights. At night, these rays can become longer and denser. Starbursts usually only affect nighttime vision, but in some cases can appear around lights at any time of the day.
The most common cause of starbursts is to do with pupil size. In darkened conditions the pupil dilates/widens, allowing light to enter the eye through both the central and peripheral cornea areas. If when Lasik eye surgery is performed the treatment zone for vision correction does not cover the area of the cornea which allows in light during darkness, then light will enter through the uncorrected peripheral cornea causing the starbursting affect.
The very wide Optical Zone Treatment of the Amaris decreases dramatically the Starbursting effect.
5- The Turbo Eye Tracker:
The eyes can involuntarily move an average of 90 micrometers per millisecond in all directions for a short time while fixating on the laser light. Therefore, exact centering and constant positioning of the eye is of decisive importance for a successful eye laser treatment.
The SCHWIND AMARIS’ Eye Tracker camera monitors the position of the eye with approximately 1050 measurements per second. The total reaction time of the SCHWIND AMARIS laser systems is only three thousandths of a second (3 millisecond).
So for the patients, they must understand that even when their eye moves in 6 Dimensions, the Amaris Eye Tracker will move 2 milliseconds after their eye movement (in any direction) and this Turbo Eye Tracker movement is faster than the laser itself. The Eye Tracker speed is 1050 Hertz.
Tracking 6 Dimensions movement of the eyes is only available in your 750 S Amaris.
6- Clinical Studies and over 50 years of German Engineering from Schwind:
A multitude of clinical studies proves the excellent treatment quality with the SCHWIND AMARIS technology. A current example is a clinical study with 1,000 eyes carried out recently by the worldwide renowned Shinagawa LASIK Center in Tokyo, Japan. The mean preoperative spherical equivalent (SEQ) and astigmatism were -5.10 ±2.31 D and -1.02 ±0.85 D.
